Job Description
The New Braunfels Fire Department has served the citizens of New Braunfels for over 130 years and is one of the oldest fire departments in the state of Texas. We are seeking an empathetic, resourceful, and communicative individual to join our team in this innovative position. You will continue to successfully serve the current and future needs of the New Braunfels community. Mobile Integrated Health aims to serve within the gaps found in emergency response systems, often prevalent among the most vulnerable populations. This position also works to relieve stress on the EMS, law enforcement, and emergency department systems that are frequently over-utilized. Under general supervision, the Mobile Integrated Healthcare (MIH) Paramedic will assist and navigate citizens to health and human resources to help resolve their healthcare problems in an attempt to reduce non-emergency 9-1-1 calls and enhance a citizen's quality of life.
Provide comprehensive patient care coordination and case management services within the Mobile Integrated Health (MIH) Program, including daily triage of referrals, management of high-utilizer patients, and ongoing continuity of care. Conduct in-person patient visits; complete medical assessments, preventive health education, documentation, care plans, referrals, and follow-up coordination. Perform extensive pre-visit research to identify appropriate medical, behavioral health, and social service resources; evaluate Program eligibility and curate patient-specific referrals to reduce barriers to care. Coordinate care with physicians, behavioral health providers, home health agencies, community organizations, and social service partners. Support opioid misuse prevention and harm-reduction efforts by providing patient education on overdose risks, warning signs, and prevention strategies; distributing medication disposal resources; assisting with access to naloxone when appropriate; and referring individuals to licensed community providers and specialized treatment resources as needed. Assist patients with navigating managed healthcare plans, obtaining medications, and completing applications for insurance, disability benefits, and financial assistance Programs. Maintain accurate, confidential patient records; ensure documentation compliance, data integrity, and quality assurance. Track, compile, verify, and report Program data, including emergency services utilization and high-utilizer tracking; prepare and submit required monthly reports to the City Manager's Office. Support Program operations and initiatives and ensure compliance with City policies and procedures. Operate, inspect, and maintain Fire Department vehicles and equipment; report repair needs as required. Serve as a liaison between patients, Fire Department personnel, community partners, and external stakeholders to support continuity of care and Program awareness. Represent the MIH Program at community meetings, outreach events, and professional forums; deliver presentations to community partners, City departments, and other agencies regarding MIH services and outcomes. Provide guidance and information to external organizations and fire departments interested in developing MIH or community health Programs. Perform specialized duties as a Child Passenger Safety Technician, including scheduling, documentation, referrals, and continuing education. Participate in required meetings, training, and professional development to maintain certifications and remain current with MIH best practices. Perform other duties as assigned.
